    From my lived experience, if your insurance company requires you to use generic brands of prograf/tacrolimus, then you MUST ask your pharmacy to guarantee that each and every month they will dispense tacrolimus from the same manufacturer as your daily dosage is based on serum blood levels and CONSISTENCY is critical to ENSURE therapeutic doses that protect your heart.

    If your pharmacy will not/can not guarantee this, then it is imperative that your transplant team write for trade name Prograf as MEDICALLY NECESSARY as a matter of protecting your heart. Insurance can NOT stand against a medically necessary prescription, nor can they argue cost savings as it is FAR more costly in the long run to deny brand name program.
